- Educational Opportunity Application - Must be submitted at least ten days in advance of the start of the opportunity to be approved. If it is received after this time, it will be denied.
- UCPS Absence Reporting Form *Absence notes are only submitted online." Parents/guardians must set up an account to report an absence from the previous day or current day due to illness. Parents can also upload medical/court documents using this system. NOTE: The email used to create the account must match the parent email on record with the school or submitted absences will not be processed. Please view the Absence Reporting Guide (Spanish version) to answer questions.
